A health plan that uses MultiPlan or PHCS may cover drug and alcohol rehab in California, but the logo alone does not confirm your benefits. Your insurer or plan administrator must check the treatment, facility and approval requirements. MultiPlan and PHCS are provider networks; they do not provide insurance or pay your treatment claims. [1]
If you are looking for help in Los Angeles, you can check your insurance for rehab with Avedis Recovery. A benefits review can help you understand which services your plan includes and what you may need to pay before making an admission decision.
What MultiPlan and PHCS Mean on Your Insurance Card
Your card may display several names because the organizations involved have different jobs. The employer or insurer provides the health plan, an administrator may handle its benefits and claims, and a provider network helps connect the plan with participating clinicians and facilities.
PHCS is a national primary PPO network. The MultiPlan Network is a complementary PPO network that a health plan may use for additional provider access. Ask which network and benefit level apply to your proposed treatment; the two names are not interchangeable for every plan. [3] [4]
You may also see the name Claritev. MultiPlan Corporation announced its corporate rebrand to Claritev in February 2025. The announcement said existing offerings would not be affected. [2]
Why Two Cards With the Same Logo Can Have Different Benefits
Network access can be configured for different kinds of plans, including limited benefit arrangements. A card with a familiar network logo is therefore not enough to establish comprehensive rehab coverage. [3]
Ask for your Summary of Benefits and Coverage and the plan document or Evidence of Coverage. The summary helps you compare benefits and costs; the full plan terms explain the details and exclusions. [13] If your coverage is a fixed indemnity product, it may pay a set cash amount rather than cover the remaining treatment bill. Fixed indemnity coverage is not a substitute for comprehensive health insurance. [16]
Which Rehab Services Could Your Plan Cover?
Marketplace health plans include substance use disorder treatment and mental health services among their essential health benefits. That rule does not mean every product using a MultiPlan or PHCS network offers identical coverage. The questions below help you check the services your treatment team recommends. [5]
Medical Detox
Medical detox provides supervised care during withdrawal. Ask whether the recommended setting is covered, whether a planned admission needs approval, and whether clinician visits, medications and laboratory work are included in the estimate or billed separately. Give the plan the actual facility details rather than asking only whether “detox” is covered.
Residential Treatment
Residential treatment involves living at the facility while receiving structured care. Ask the plan to check residential substance use treatment specifically. Coverage for an inpatient hospital admission does not, by itself, answer how the plan will process a residential rehab stay. Clarify the approved setting and dates before arranging admission.
Partial Hospitalization and Intensive Outpatient Care
A partial hospitalization program, or PHP, provides structured treatment without an overnight residential stay. An intensive outpatient program, or IOP, provides scheduled treatment while you live outside the facility. Ask whether your benefits distinguish these programs from ordinary outpatient therapy and whether each requires separate authorization.
Co-Occurring Mental Health Care and Medications
If you need treatment for both substance use and a mental health condition, include both needs in the benefits discussion. Ask about psychiatric appointments, therapy and medication management. Also check whether prescribed medications are paid through the treatment benefit or a separate pharmacy benefit. A general answer about “behavioral health” may leave these costs unclear.
How Much Will You Pay for Rehab With a MultiPlan or PHCS Plan?
Your estimate needs the plan’s allowed amounts and cost-sharing rules. A treatment center’s advertised price alone cannot tell you what your share will be. Coinsurance is generally calculated as a percentage of the allowed amount after the applicable deductible. [7]
| ITEM TO CHECK | WHAT TO ASK |
|---|---|
| Remaining deductible | How much of the applicable deductible is still unpaid for this plan year? |
| Copay or coinsurance | Is my share a fixed amount, a percentage, or different for different services? |
| Benefit tier | Will this facility and service be processed at in-network or out-of-network rates? |
| Out-of-pocket limit | Which charges count, and how much qualifying spending remains? |
| Separate charges | Are medications, labs, individual clinicians, housing or optional services billed separately? |
A Simple Cost Example
Suppose covered in-network services have an allowed amount of $10,000. You have $1,000 left on the applicable deductible, followed by 20% coinsurance. In this simplified example, you pay the $1,000 deductible plus 20% of the remaining $9,000, or $1,800. Your total is $2,800. [7]
This is an illustration, not an Avedis price or a standard MultiPlan benefit. It assumes all services are covered, the same coinsurance applies throughout, there are no separate charges, and you do not reach your remaining out-of-pocket limit first.
The in-network out-of-pocket maximum generally applies to qualifying covered care. Premiums, noncovered services and out-of-network spending are not included in that in-network limit. Ask about any separate out-of-network protection instead of assuming the same cap applies. [8]
How to Find Rehab in California With Your Network
Start with the provider directory recommended by your health plan. Claritev also offers a provider search, but a listing is only the starting point. Confirm the result with the plan and the treatment center, especially if the address or program differs from what you intend to use. [1]
When speaking with admissions, identify the exact facility and level of care. A useful question is: “For my plan, will treatment at this location be processed as in-network, out-of-network, or through another discount arrangement?”
Also ask whether the facility’s clinicians bill separately and whether the same benefit applies if you move from residential care to PHP or IOP. This gives you a more useful answer than a general statement that a center “takes MultiPlan.”
Avedis lists MultiPlan on its admissions page and offers treatment in Los Angeles. Its admissions team can check how your specific policy may apply to the proposed Avedis facility and services. [15]
How to Check Your Benefits Before Admission
Have both sides of your insurance card available. Look for the member services number and the company responsible for your benefits. Claritev directs providers to the number on the card to verify eligibility and authorization requirements. [4]
Ask for a review that answers these questions:
Is coverage active for the intended treatment dates, and who administers the substance use treatment benefit?
Does the plan cover the recommended level of care at this specific facility?
What authorization or referral is required, and who will request it?
What deductible, copays or coinsurance apply, and what expenses are outside the estimate?
Can I receive the findings in writing, including the date of the check and a call reference number?
Avedis’s insurance verification page explains what information to provide and how the team checks your benefits. Verification is free and does not commit you to treatment. [14]
Verification and Prior Authorization Are Different
Verification checks the benefits available under the policy. Prior authorization is the plan’s review of whether a proposed service meets its requirements for medical necessity. A plan may require that approval before certain nonemergency services. Even prior authorization is not a promise that every charge will be paid. [6]
Before a planned admission, ask which approvals are complete, which are still pending, and whether your estimate could change. If you need emergency care, seek it immediately rather than waiting for a routine insurance check.
How Long Will Insurance Pay for Inpatient Rehab?
There is no single covered length of stay for everyone with a MultiPlan or PHCS logo on their card. Ask about the dates or number of days actually authorized for your proposed care. A treatment recommendation and an insurance approval are different pieces of information.
If the plan reviews a stay in stages, ask when the next review occurs, what clinical information is needed and who handles the submission. Also ask how the team will tell you about a change in coverage or an expected change in your costs. An advertised 30-, 60- or 90-day program does not establish your individual benefit.
What If Your Plan Limits or Denies Coverage?
Request the written reason, the relevant plan provision and the instructions for appeal. Ask whether the issue concerns missing information, the treatment setting, network status or medical necessity. Keep copies of the decision, benefit documents and correspondence. California’s Department of Insurance explains the complaint and independent medical review options for policies it regulates. [11]
California Protections Depend on the Type of Plan
California-regulated commercial health plans and insurers must cover medically necessary mental health and substance use disorder treatment under state law. California guidance also addresses arranging out-of-network treatment when appropriate medically necessary services are unavailable in the network. Ask the plan about that process before arranging nonemergency care yourself. [9]
Self-funded employer plans follow a different regulatory route. Many private employer plans are regulated under federal law through the U.S. Department of Labor. Receiving treatment in California does not automatically make a plan subject to California insurance rules. Your employer’s benefits team or plan administrator can help identify which rules apply. [10]
For a DMHC-regulated plan, the DMHC grievance guidance explains when to complain to the plan and when to contact the Help Center. Urgent situations have exceptions to the usual waiting process. [12]

Frequently Asked Questions
Not necessarily. Covered treatment can still leave you with a deductible, copays or coinsurance. Reaching the applicable out-of-pocket limit does not turn excluded services into covered benefits. Request an estimate for the actual treatment plan. [8]
